The Username you type here will be used to identify you for online account information. This allows access to your account info for billing and configuration. Your "Username" must be at least 5 characters long, only (a-z and 0-9). It will be coverted to all lowercase. 		Please type a username:

After clicking on the "Process Application Now" button, the information you have provided will be
sent to our signup server. There the information will be used to create a printable form which you
should sign and return to:

or you can expedite your request by faxing it to:

Your application will not be considered until you send a signed copy of the application. When
received, a site surveyor will contact you and check your location for available service. You are
submitting a contract, you have three business days after we contact you to cancel the contract.
The full force of this agreement is pending the availability of service at your location.
